At the beginning of his career he formed a comic duo with another stand-up comedian, Ciro Maggio, the couple was called Bibì & Cocò. The duo was famous in the Neapolitan environment thanks to appearances on local television stations, and for the production of cassette tapes with comic-idiotic songs. This popularity led them to be called to perform very often during the wedding ceremonies of many Neapolitan couples, often attending several ceremonies during the same day. Biagio then achieved success with TeleGaribaldi, on the Campania regional broadcaster Channel 9, and later with Pirati and Pirati Show, on TeleNapoli34, but he had already made himself known nationally with the Macao program, conducted by Alba Parietti. In that period, after the experience of Telegaribaldi, he opened the "cabaret Portalba" (formerly the Bellini Theater). In the early 2000s he was also present in the Christmas films with Boldi and De Sica Body Guards - Guardie del corpo (2000), Merry Christmas (2001), Natale sul Nilo (2002), Natale in India (2003), and participated often on the show Buona Domenica.
From then on, there is the rise of Izzo in cinema and television. In fact, his appearances on the big screen have intensified more and more. In 2004, he participated in the film In questo mondo di ladri, together with Carlo Buccirosso and Ricky Tognazzi, under the direction of Carlo Vanzina. In 2005 he appeared in the film Cose da pazzi together with Vincenzo Salemme and Maurizio Casagrande, under the direction of Salemme himself. In 2007 he obtained his first conduct at a national level, being he in charge of the Stasera mi butto program, together with Caterina Balivo (with whom he led Miss Italy in the world the following year).
In 2008 he was part of the cast of the program Volami nel cuore, and returned to work with Boldi in the films Matrimonio alle Bahamas (2007) and La fidanzata di papà (2008). He also began a theatrical career, always of a comic character, in which he can be seen under the direction of Claudio Insegno and Pino Insegno in Tutto per Eva, solo per Eva, C’è un uomo nudo in casa, Due comici in Paradiso, Il re di New York, Una pillola per piacere, Un tè per tre, Guardami guardami, Tutti con me, Esse o esse, Come un Cenerentolo, all works written by Bruno Tabacchini and Biagio Izzo. In 2010 he was in the fixed cast of the Rai 2 program Il più grande (italiano di tutti i tempi), conducted by Francesco Facchinetti. In 2012 he joined Paola Perego in the Rai 1 program Attenti a quel due - La sfida. In 2012 he joined Maurizio Casagrande and Pino Insegno in the film Una donna per la vita, and also joined Massimo Boldi and Maurizio Mattioli in the TV film Natale a 4 zampe.
